1. If one
forgot to add the Al HaNisim passage after Modim in the Amida but before he/she said
Hashem's name in the beracha following Al HaNisim, which is "Hatov Shimcha
U'Lecha Naeh LeHodot," then he/she can go back to say Al HaNisim and
continue regularly. However, if one said the beracha after Al HaNisim completely,
then he/she cannot go back, rather at the end of the Amida he/she before
the last Yehiyu LeRatzon (before Oseh Shalom) says "Modim anachnu lefanecha Al HaNisim..."
until the end of the paragraph and continues straight where he/she left off(at Yehiyu LeRatzon and Oseh Shalom). If one realizes he/she forgot
while literally saying Baruch Atah Hashem after Modim, he/she may not end it with
Lamdeni Chukecha (which is as if saying a pasuk from tehillim and not a
beracha and therefore would make the person think they can go back to say Al HaNisim regularly). In
this situation, he/she must say the rest of the beracha and say Al
HaNisim like in the case where one forgot after he said Hashem's name as
mentioned before (before the final Yehiyu LeRatzon). If one forgot altogether the Al Hanisim addition, they do not return to repeat the Amida.
2. It is correct to say "VeAl HaNisim" with the addition of the letter 'Vav' in Hannukah and Purim. When one adds the letter 'Vav' they declare that it is as if they are continuing the Modim Prayer in the Amida and continue to thank Hashem for all the miracles, wonders, etc.
