1. The Ba'al Koreh does not need to open up the Megillah before saying the Brachot for Kriyat HaMegillah, rather says the brachot and then opens up the Megillah and begins to read. When he finishes the first page, he should not roll the page like a Sefer Torah but needs to leave it open flat so that when he's done reading, the entire Megillah is rolled out for public view. When reading the Megillah, one should be cautious that they are not rolling out the pages onto the floor and after the Kriya is over, the Megillah is rolled from end to beginning so that it is opened to the first page and not the last. After the Kriya, one should roll up the Megillah completely in order to prevent disrespect to the sacred text to be left opened for no reason, and then say the Brachot needed to be said after the Kriya. One should be stringent and do netilat yadayim (without bracha) before touching the Megillah because some say it is as if touching a Sefer Torah which cannot be touched by your bear skin.
2. The Ba'al Koreh needs to specify and be clear in his reading of the Megillah and also the lsitener needs to hear every word. Even if a single word was not heard, the listener did not fulfill his obligation of Kriyat HaMegillah. Therefore, when the crowd makes noise when Haman HaRasha's name is mentioned, the Ba'al Koreh should wait pateintly untill there is silent to continue. It is best for all listeners to have a Kosher Megillah so they can read and follow the Ba'al Koreh by whispering, BUT one who does not have a Kosher Megillah should not follow the Koreh in a whisper and should listen carefully silently.
